cut out images? How do?

I have seen some gorgeous cards lately that have the image cut out on the edges. It looks like the image is stamped, then the whole piece of CS is trimmed, then part of the image is cut out so it appears as if it's sitting on top of the paper. How are y'all doing that? Just the way I think? How long does it take to do something like that?
