My local stamp store has a customer desperately seeking this die. They think it is by Sizzix.

When I worked at my LSS many years ago we had die cuts like this, they came already precut from manufacturers, this was before home die cutting machines became popular, I'm afraid this is probably one of those and you might not find the exact one today.
